Recall History: the Fresenius Liberty Cycler

When a medical device shows evidence of a defect or possible malfunction that could cause harm to a patient, the FDA issues a recall to ensure any potential issues are remedied and protect consumer safety. At this time, the FDA has issued four recalls regarding the Fresenius Liberty Cycler home dialysis machine:

September 2010: 370,588 Fresenius Liberty Cycler home dialysis units were recalled when it was determined that scratches to the device may cause cassettes to leak, leading to potentially contamination of dialysis fluids.

February 2013: A recall was issued to address a potential software malfunction that could lead to device overfill with dialysis fluids. According to the FDA recall notice, there was “a small risk of a leak in Cycler Sets and if the Cycler Set is punctured, the dialysate may become contaminated potentially resulting in peritonitis.”

July 2013: This recall also regards the potential for leaks in the Liberty Cycler. Specifically, the FDA stated that the cassettes in the device “may leak during set up or during peritoneal dialysis treatment.”

November 2013: On November 5, 2013, the Fresenius Liberty Cycler was the subject of a third recall regarding potential leaking. The FDA issued this recall because “fluid leaking may be observed inside the pump compartment of the Liberty Cycler and may result in dialysate becoming contaminated, potentially causing peritonitis.”

Each of these Fresenius Liberty Cycler recalls were issued to address alleged defects which could lead to contamination and the onset of peritonitis. Peritonitis is a serious infection of the peritoneum, the lining of the abdominal cavity and organs. Even with prompt medical intervention in the form of the administration of antibiotics and possible surgery, peritonitis may prove fatal.
